WELCOME TO YOUR RESIDENTIAL/OUTPATIENT SERVICES INTAKE!
RESIDENTIAL TREATMENT INTAKE
These are the items that you will need to have before you enter into treatment at Progress House.
1. Secure funding
2. Gather needed documents - ID, proof of pregnancy/live birth, TB test proof/results,
3. No substance use within last 24 hours.
If you are a MediCal Client:
1. Contact the County in which you receive MediCal and begin the process to enter treatment.
2. Receive an Assessment for Services with the County.
3. Secure funding from the County for treatment.
4. Secure needed documents - ID, MediCal Card, proof of pregnancy/live birth, TB test.
5. No substance use within last 24 hours.
OUTPATIENT TREATMENT INTAKE
These are the items you will need before you intake at Progress House outpatient services:
1. Secure funding (outside agency or provider funding)
2. Proof of MediCal
3. No substance use within last 30 days.

IF I'M GOING INTO RESIDENTIAL TREATMENT - WHAT DO I BRING
Limited storage space is available, please keep personal belongings to a minimum. Only one weeks of clothing will be accepted into the facility. All client belongings, will be searched at intake. Under NO circumstances are alcohol, drugs, paraphernalia or weapons allowed on Progress House property. We cannot accept responsibility for any personal property lost, misplaced, stolen or damaged, which you have not given over for safekeeping.
Please Remember to Bring
- MediCal Information & Card (If Applicable)
- Current TB Test
- ID – Driver’s License, Identification Card, or other photo ID.
- Bank card & Emergency contact numbers
- Postage Stamps/Envelopes
- Female Clients – Sanitary Pads or Napkins, NO TAMPONS as we are on septic systems.
- Allowable Personal Hygiene Products Unopened:
- Soap, Lotion, Shampoo, & Conditioner.
- Razors & Deodorant.
- Hair Gel.
- Medications:
- Only bring prescription medications you are currently taking under doctor’s supervision.
- All medications/prescriptions must be turned over to Progress House staff on admission
- Prescription medications must be in the original pharmacy container with your name on the prescription label – any medications that do not have your name on the bottle will be disposed of. Any over the counter medications, if they are approved, will be stored with your prescription medications
Suggested Clothing List:
- Jeans/pants/shorts
- Shirts – no bare midriff's or halter tops are allowed
- Undergarments – bra, underwear, socks, etc.
- Sweatshirts or sweaters
- Coat/jacket
- Seasonally appropriate outer wear including boots, coats, hats for winter
- Shoes – such as sneakers, slippers, sandals. You will be required to wear foot wear on your feet at all times.
- Night wear such as pajamas will be required to wear during sleep hours.
- Bathing suit
- Fitness/Exercise clothing.
- Miscellaneous Items:
- Address book
- Recovery based books
- Photos
- Journal/Notebook/ Stationary
- Mp3 player (with no internet access), or CD player (with earphones). Music with excessive profanity will not be permitted.
Do Not Bring:
Clothing sexually suggestive or revealing, or suggestive of alcohol, drugs, gambling, etc.
- Cell phone
- Electronics like laptop, iPad/iTouch, portable TV, stereo, video games, etc
- Tobacco products with alcohol dipped flavor
- Vaping products of any kind
- All hygiene must be new and unopened
- Mouthwash containing alcohol
- Highly scented toiletries
- Aerosols of any sort
- Weapons, including pocket knives
- Valuable items/jewelry
